All employees choosing to receive the COVID-19 vaccine will need to pre-schedule a time to receive the vaccine. Consent and authorizations will be collected at the time of the employee’s scheduled vaccination as part of government regulatory reporting requirements. Vaccines are strongly encouraged, but not required at this time.
Scheduling for vaccinations will go live at 7:00 a.m. Thursday, December 17, 2020. All identified employees will need to call 612-262-5533 to schedule an appointment. This phone will be answered by the Customer Experience Center Monday through Thursday, 7:00 a.m. to 8:00 p.m.; Friday, 7:00 a.m. to 5:00 p.m.; and Saturday and Sunday, 8:00 a.m. to 6:00 p.m. Employees should schedule their vaccination appointment within their slotted week and leaders will provide further guidance on the specific day during each week (e.g., Tuesday, Wednesday, Thursday). Appointments are on a first come, first served basis.
Employees must get their vaccination at their home site location (e.g., United Hospital employees must get their first and second vaccine at United Hospital). Leaders within EMS and Home Care will direct employees on where their “home location” is for vaccination.

EMPLOYEE INSTRUCTIONS
· Employees must register and schedule their vaccine appointment by call the CEC at 612-262-5533.
· Employees must consent for the vaccine record to be in Excellian and for Allina Health to report to the state about the vaccine. Failure to provide consent means that the employee cannot get the vaccine at this time.
· Employees may not walk-in to the COVID-19 vaccine clinic.
· Employees’ managers will provide guidelines as to when they should schedule themselves during the designated week (e.g., Tuesday, Wednesday, Thursday).
· Vaccines are only given at designated COVID-19 clinic times.
· Employees must get their vaccine at their home site location (e.g., United Hospital employees must get their first and second vaccine at United Hospital).
· Employees must ensure that they will be available to take the second dose exactly 21 days from the first dose.
· If an employee is ill on the scheduled day of their vaccine, the employee should follow the regular call-in procedure at work. The employee’s leader will follow up with the employee on rescheduling a vaccine time.
· On the day of an employee’s scheduled vaccine, the employee should let their leader know about the need to leave the department to receive the vaccine so the leader can arrange for coverage.
